The World Health Organization (WHO) calculates that 12.6 million premature deaths per year are attributable to unhealthy environments, 8.2 million of which are due to non-communicable disease, with CVD (including stroke) being the largest contributor, accounting for nearly 5 million of these deaths.2 Among all environmental pollutants, poor air quality is the most important risk factor, and ambient air pollution due to particulate matter <2.5 µm (PM2.5) exposure ranks 5th among all global risk factors in 2015, leading to 4.2 million deaths annually as estimated by the Global Burden of Disease study.3 Nine out of 10 people worldwide are exposed to ambient air pollutant levels above WHO guidelines (>10 µg/m).3,4 Using a novel exposure-response hazard function (global estimate of exposure mortality model) to estimate global mortality attributable to air pollution, Burnett et al.5 and Lelieveld et al.6 found that around 9 million global premature deaths (790 000 excess deaths in Europe alone) were attributable to air pollution,7 numbers that are well comparable to that of smoking.6 These figures are substantially higher than those estimated by the WHO and Global Burden of Disease study.2,3Ambient noise is the other omnipresent exposure with emerging data suggesting a large attributable burden of disability to this factor in many urban environments.

In Western Europe, it is estimated that around 1.6 million healthy life years are lost every year due to noise. It is estimated that a large part of the European population is exposed to noise originating from road traffic at levels exceeding 55 decibels [dB(A), A-weighted decibel scale adapted to the human how to get a prescription for zithromax hearing frequencies]. 20% exposed to levels exceeding 65 dB(A) during the daytime. The important role of oxidative stress and inflammation for noise-induced cardiovascular complications was also supported by changes of the plasma proteome, centred on redox, pro-thrombotic and proinflammatory pathways, in subjects exposed to train noise for one night [mean SPL 54 dB(A)].30 Pathophysiology and epidemiology of air pollution and cardiovascular diseaseSince the publication of an American Heart Association Scientific Statement,31 there has been a consistent stream of epidemiological and mechanistic evidence linking PM2.5, the most frequently implicated air pollution component with CVD.5,6 Mounting evidence suggests that health risks attributable to PM2.5 persist even at low levels, below WHO air quality guidelines and European standards (annual levels <10 and <25 µg/m3, respectively). Updated exposure-response dose curves suggest a robust supralinear concentration-response-curve for PM and CVD with no apparent safe threshold level.32 EpidemiologyCurrent estimates suggest air pollution is associated with around 9 million how to get a prescription for zithromax premature deaths, worldwide annually with ∼40–60% of mortality attributed to cardiovascular causes.5,33Short-term exposure (over hours or days) is associated with increased risk for myocardial infarction, stroke, heart failure, arrhythmia, and sudden death by about 1–2% per 10 µg/m3. (i) Active personal exposure mitigation with home air cleaning and personal equipment (Table 2). However, the use of medications for primary and secondary prevention of how to get a prescription for zithromax CHD should be encouraged if indicated for other reasons. Housing and urban design to improve cardiovascular healthTwo-third of the European population live in urban areas and this number continues to grow. A recent Statement on Air Quality Policy has discussed aspects how to get a prescription for zithromax in the built environment that may be targeted in order to reduce exposures to PM2.5 (in press 2020).

Briefly, built environment features may directly or indirectly modify adverse cardiovascular effects of air pollution through the indoor living environment, green spaces, roads, utilities, and transportation infrastructure. The design of communities has the potential of impacting exposures, by affecting the continuum of human existence across indoor living, commuting, working, how to get a prescription for zithromax and recreation (Figure 3). The layout of roads, sidewalks, green spaces, and the availability of cheap public transportation can affect travel behaviour and can help alleviate air quality.39 Communities with proximity and compactness have been associated with higher life expectancy, improved air quality, and health.40,41 Green environments can improve air quality, encourage physical activity, and promote social interactions, ultimately improving cardiovascular health. Indeed, there is evidence to support a how to get a prescription for zithromax protective association of green spaces on PM-associated CVD.42,43All-cause and ischaemic heart disease mortality related to income deprivation has been shown to be lower in populations who live in the greenest areas, vs. Those who have less exposure to green space.44 Recently, Giles-Corti identified eight integrated regional and local interventions that, when combined, encourage walking, cycling and public transport use, while reducing private motor vehicle use.45 These eight interventions are directed to reduce traffic exposure, to reduce air pollution and noise, and to reduce the important public health issue loneliness and social isolation, to improve the safety from crime, to reduce physical inactivity and prolonged sitting, and to prevent the consumption of unhealthy diets.45 Figure 3Urban design considerations to reduce exposure to noise and air pollution.Figure 3Urban design considerations to reduce exposure to noise and air pollution.

Future perspectives. Opportunities and challenges over the next decadeEfforts to mitigate air pollution and noise are endeavours that involve complex economic and geopolitical considerations. Measures such as transportation how to get a prescription for zithromax reform, shift to zero-emission fuels, urban landscape reform, and ecologically sound lifestyle changes may help simultaneously alleviate air/noise pollution while accomplishing climate change goals. However, reducing air pollution and noise may have short-term challenges due to economic incentives that are substantially misaligned with health and environmental priorities and thus opportunities to understand the importance of these factors in human health will sadly continue.

An important avenue of investigation is convergent studies that look at the broad and collective impact and burden of how to get a prescription for zithromax air and noise pollution as archetypal environmental risk factors. The questions that need to be addressed are many and include the magnitude and time course of response of co-exposure, interactive effects of environmental factors on surrogate measures, duration of effect/time course of reversal, impact on circadian rhythm, and finally the effect of reversal as well as prevention and lifestyle approaches that may help mitigate risk (e.g. Diet, stress, and exercise).The rapid development of personalized technologies that provide multiple measures of health in fine temporal detail in conjunction with data on environmental exposure provide an unprecedented opportunity for research and may allow an extraordinary understanding of the interactions how to get a prescription for zithromax between environmental and non-environmental risk factors over long durations. Together with developments in next-generation sequencing technologies, and opportunities in big data, assimilative studies of this nature may finally provide a granular view of the environmental–genetic interactions leading to the development of CVD. However, the extent of these how to get a prescription for zithromax advances may be tempered by the need to manage subject burden and costs, and imprecise data on many environmental variables.

Increased awareness of the societal burden posed by environmental risk factors and acknowledgement in traditional risk factor guidelines may pressurize politicians to intensify the efforts required for effective legislation.The cardiovascular community has a responsibility to help promulgate the impact of, not only health lifestyle and diet, but also over the outsize impact of air and noise pollution on cardiovascular health. Individuals can apply political pressure through democratic means and lobbying to enact changes at regional and national levels that lead to reductions in noise/air pollution exposure. Patient organization how to get a prescription for zithromax can provide a strong voice in the call for action at governmental level. Importantly, air pollution was mentioned in the published guidelines for cardiovascular prevention, but the recommendations to reduce pollution were completely insufficient,47 while prevention measures with respect to traffic noise were completely lacking. Noise and air pollution represent significant cardiovascular risk factors, it is important that these factors are included into the ESC guidelines, and others, for myocardial infarction, arterial hypertension, and how to get a prescription for zithromax heart failure.

Indeed, what the optimal timing of staged PCI is has not buy zithromax online no prescription been completely defined. Moreover, assessment of intermediate non-culprit lesions still represents a clinical conundrum, as pressure-wire indexes do not seem able to correctly identify those patients in whom deferral is safe. The correlated pathology at the myocardial level is cardiomyocyte necrosis, measured by troponin release, or, less frequently, myocardial ischaemia without cell damage (unstable angina).(2) Troponin assaysHigh-sensitivity troponin assay (hs-cTn) measurements are recommended over less sensitive ones. However, many cardiac pathologies other than MI may also result in cardiac troponin elevations.(3) Rapid ‘rule-in’ and ‘rule-out’ algorithmsIt is recommended to use the 0 h/1 h algorithm (best option) or the 0 h/2 h algorithm.

Used in conjunction with clinical and ECG findings, the 0 h/1 h and 0 h/2 h hs-cTn algorithms allow identification of appropriate candidates for early discharge and outpatient management.(4) Ischaemic/bleeding risk assessmentInitial hs-cTn levels add prognostic information in terms of short- and long-term mortality to clinical and ECG variables. The Global Registry of Acute Coronary Events (GRACE) risk score is superior to (subjective) physician assessment for the occurrence of death or MI. The Academic Research Consortium-High Bleeding Risk may be used to assess the bleeding risk.(5) Non-invasive imagingEven after the rule-out of MI, elective non-invasive or invasive imaging may be indicated according to clinical assessment.

Coronary computed tomography angiography or stress imaging may be options based on risk assessment.(6) Risk stratification for an invasive approachAn early routine invasive approach within 24 h of admission is recommended for Non ST segment elevation myocardial infarction (NSTEMI) based on hs-cTn measurements, GRACE risk score >140, and dynamic new or presumably new ST-segment changes. Immediate invasive angiography is required in highly unstable patients according to hemodynamic status, arrhythmias, acute heart failure, or persistent chest pain. In all other clinical presentations, a selective invasive approach may be performed according to non-invasive testing or clinical risk assessment.(7) Revascularization strategiesRadial access is recommended as the preferred approach in NSTE-ACS patients undergoing invasive assessment.

Percutaneous coronary intervention of the culprit lesion is the treatment of choice. In multivessel disease, timing and completeness of revascularization should be decided according to the functional relevance of stenoses, age, general patient condition, comorbidities, and left ventricular function.(8) MINOCAMyocardial infarction with non-obstructive coronary arteries incorporates a heterogeneous group of underlying causes that may involve both coronary and non-coronary pathological conditions. Cardiac magnetic resonance imaging is one of the key diagnostic tools as it allows to identify the underlying cause in the majority of patients.(9) Post-treatment antiplatelet therapyDual antiplatelet therapy consisting of a potent P2Y12 receptor inhibitor in addition to aspirin is generally recommended for 12 months unless there are contraindications.

Dual antiplatelet therapy duration can be shortened (<12 months), extended (>12 months), or modified by switching DAPT or de-escalation depending on individual clinical judgement driven by ischaemic and bleeding risk.(10) Triple antithrombotic therapyNon-vitamin K oral anticoagulants (NOACs) are preferred over vitamin K antagonists in patients undergoing PCI with an indication for long-term oral anticoagulation. Dual antithrombotic therapy with a NOAC and single antiplatelet therapy is recommended as the default strategy up to 12 months after a short period of up to 1 week of TAT. Triple antithrombotic therapy may be prolonged up to 1 month when the ischaemic risk outweighs the bleeding risk..

The primary outcome was all-cause death.

TWILIGHT-ACS’, Roxana Mehran from Mount Sinai School of Medicine in New York, USA and colleagues determined the effect of ticagrelor monotherapy on clinically relevant bleeding and major ischaemic events in relation to clinical presentation with and without non-ST elevation acute coronary syndromes (NSTE-ACS) among patients undergoing percutaneous coronary intervention (PCI) with drug-eluting stents (DES).9 The authors conducted a pre-specified subgroup analysis of The Ticagrelor With Aspirin or Alone in High-Risk Patients After Coronary Intervention (TWILIGHT) trial, which enrolled 9006 patients with high-risk features undergoing PCI with DES. After 3 months of dual antiplatelet therapy (DAPT) with ticagrelor plus aspirin, 7119 adherent and event-free patients were how to get a prescription for zithromax randomized in a double-blind manner to ticagrelor plus placebo vs. Ticagrelor plus aspirin for 12 months. Caused by coronary plaque erosion, account for approximately one-third of ACS cases. However, the underlying pathophysiological mechanisms as compared with ACS caused by a ruptured fibrous cap (RFC) remain largely undefined.11–14 In a clinical research article entitled ‘Differential immunological signature at the culprit site distinguishes acute coronary syndrome with intact from acute coronary syndrome with ruptured fibrous cap. Results from the prospective translational OPTICO-ACS study’, David Leistner from the Charite Universitatsmedizin Berlin in Germany and colleagues compared the how to get a prescription for zithromax microenvironment of culprit lesions (CLs) with IFC vs. Those with RFC.15 The CL of 170 consecutive ACS patients was investigated by optical coherence tomography (OCT) and simultaneous immunophenotyping by flow cytometric analysis as well as by effector molecule concentration measurements across the CL.

Within the study cohort, IFC CLs caused 25% of ACS while RFC CLs caused the remaining 75%, how to get a prescription for zithromax as determined and validated by two independent OCT core laboratories. But it is, perhaps, the end of the beginning.’Balance between inflammatory and reparative leucocytes allows optimal healing after MI.17 In a clinical research article ‘Molecular imaging-guided repair after acute myocardial infarction by targeting the chemokine receptor CXCR4’, Annika Hess from the Hannover Medical School in Germany and colleagues aimed to characterize infarct chemokine CXC receptor 4 (CXCR4) expression using positron emission how to get a prescription for zithromax tomography (PET) and establish its relationship to cardiac outcome. The authors tested whether image-guided early CXCR4-directed therapy attenuates chronic dysfunction.18 A total of 180 mice underwent coronary ligation or sham surgery and serial PET imaging over 7 days.

Infarct CXCR4 content was significantly higher over 3 days after MI compared with sham, confirmed by flow cytometry and histopathology. Mice that died of left ventricular (LV) rupture exhibited how to get a prescription for zithromax persistent inflammation at 3 days compared with survivors. Higher CXCR4 signal at 1 and 3 days independently predicted significantly worse functional outcome at 6 weeks assessed by cardiac magnetic resonance. CXCR4 expression correlated with contractile function.Hess and colleagues conclude that PET imaging identifies early CXCR4 up-regulation which predicts acute rupture and chronic contractile dysfunction. This supports a molecular imaging-based theranostic approach to guide therapy after MI. Gemelli IRCCS in Rome, Italy, and colleagues (including myself) note that ∼50% of patients with ST-segment elevation myocardial infarction (STEMI) have multivessel coronary artery disease, a condition associated with an increased incidence of recurrent ischaemic events and higher mortality.20,21 Based on recent evidence, a strategy of staged PCI of obstructive non-culprit lesions should be considered the gold standard for the management of these patients.22 However, several issues remain unresolved.

Indeed, what the optimal timing of staged PCI how to get a prescription for zithromax is has not been completely defined. The correlated pathology at the myocardial level is cardiomyocyte necrosis, measured by troponin release, or, less frequently, myocardial ischaemia without cell damage (unstable angina).(2) Troponin assaysHigh-sensitivity troponin assay (hs-cTn) measurements are recommended over less sensitive ones. However, many cardiac pathologies other than MI may also result in cardiac troponin elevations.(3) Rapid ‘rule-in’ and ‘rule-out’ algorithmsIt is recommended to use the 0 h/1 h algorithm (best option) or the 0 h/2 h algorithm. Used in conjunction with clinical and ECG findings, the 0 h/1 h and 0 h/2 h hs-cTn algorithms allow identification of appropriate candidates for early discharge and outpatient management.(4) Ischaemic/bleeding risk assessmentInitial hs-cTn levels add prognostic information in terms of short- and long-term mortality to clinical and ECG variables.

The Global Registry of Acute Coronary Events (GRACE) risk score is superior to (subjective) physician assessment for the occurrence of death or MI. The Academic Research Consortium-High Bleeding Risk may be used to assess the bleeding risk.(5) Non-invasive imagingEven after the rule-out of MI, elective non-invasive or invasive imaging may be indicated according to clinical assessment. Coronary computed tomography angiography or stress imaging may be options based on risk assessment.(6) Risk stratification for an invasive approachAn early routine invasive approach within 24 h of admission is recommended for Non ST segment elevation myocardial infarction (NSTEMI) based on hs-cTn measurements, GRACE risk score >140, and dynamic new or presumably new ST-segment changes. Immediate invasive angiography is required in highly unstable patients according to hemodynamic status, arrhythmias, acute heart failure, or persistent chest pain.

In all other clinical presentations, a selective invasive approach may be performed according to non-invasive testing or clinical risk assessment.(7) Revascularization strategiesRadial access is recommended as the preferred approach in NSTE-ACS patients undergoing invasive assessment. Percutaneous coronary intervention of the culprit lesion is the treatment of choice. In multivessel disease, timing and completeness of revascularization should be decided according to the functional relevance of stenoses, age, general patient condition, comorbidities, and left ventricular function.(8) MINOCAMyocardial infarction with non-obstructive coronary arteries incorporates a heterogeneous group of underlying causes that may involve both coronary and non-coronary pathological conditions. Cardiac magnetic resonance imaging is one of the key diagnostic tools as it allows to identify the underlying cause in the majority of patients.(9) Post-treatment antiplatelet therapyDual antiplatelet therapy consisting of a potent P2Y12 receptor inhibitor in addition to aspirin is generally recommended for 12 months unless there are contraindications.

Dual antiplatelet therapy duration can be shortened (<12 months), extended (>12 months), or modified by switching DAPT or de-escalation depending on individual clinical judgement driven by ischaemic and bleeding risk.(10) Triple antithrombotic therapyNon-vitamin K oral anticoagulants (NOACs) are preferred over vitamin K antagonists in patients undergoing PCI with an indication for long-term oral anticoagulation. Dual antithrombotic therapy with a NOAC and single antiplatelet therapy is recommended as the default strategy up to 12 months after a short period of up to 1 week of TAT. Triple antithrombotic therapy may be prolonged up to 1 month when the ischaemic risk outweighs the bleeding risk..
